In British education system, a doctor is a research scholar with a Ph.D. (doctor of philosophy) degree. He is called Ph.D. because he does research work on unsolved problems of that particular subject. But Americans have changed this definition as their universities award bachelors level degrees like M.D.(doctor of medicine) and J.D.(Juris Doctor).As to your original question, a doctor of medicine is called a physician if he performs physical or clinical examination, and a surgeon if he performs surgery.
